Housing Continuum/Support Services


Having a safe, stable place to call home, is a cornerstone of recovery. LifeStream offers an array of housing options to people living with mental health conditions and substance use disorders. Each program is designed to promote recovery, foster maximum independence and reduce the need for other forms of shelter, hospitalization stays or involvement with the criminal justice system.

Anthony House


Anthony House is a residential substance abuse treatment program for pregnant and post-partum women and their children who are motivated to make positive changes in their lives. Anthony House provides safe, stable living arrangements with comprehensive individualized treatment services for these women with substance use, mental illness and/or co-occurring disorders and their minor children (up to the age of 17).   Anthony House provides an environment that fosters independence and helps residents gain the life skills needed to prevent future chronic homelessness. Anthony House believes that home is where the heart is, and we strive to provide a home where our residents’ hearts can heal and grow.

Supported Housing


Supportive housing services assist persons with severe mental illness in choosing, obtaining and keeping decent, stable, affordable and permanent housing. The program emphasizes integration into the community, flexible services and indefinite support with things like grocery shopping, medical appointments, housekeeping and access to other community resources.

Hope Springs Villas


Hope Springs Villas is designated as an affordable housing complex for low-income residents. Hope Springs has 34 rental units made up of one and two bedroom apartments, with three fully handicapped accessible units. Through the partnership with the Lake County Board of County Commissioners, Hope Springs Villas is a HUD-NSP property. Twenty-three (23) apartments have been assigned HUD Section 8 Project-Based Housing Vouchers (PBV). The remaining eleven (11) apartments are used for low-income families.

Open-Door

The Open Door


The Open Door is a drop-in center for individuals and families in Lake County who are homeless, or at risk of becoming homeless. The center provides a safe, compassionate and supportive environment. The center serves as a one-stop shop, linking area homeless to desperately needed amenities and services and ultimately assisting them in securing safe, affordable housing and support. Homeless individuals and families visiting the center have access to showers and laundry facilities; mailing address/phone access; food and beverages; assistance with obtaining benefits; community resources and referrals to other agencies and on-site support services.

HOPE House


HOPE House (Housing Options for Personal Enrichment) is an 8-bed time-limited permanent supported housing program for homeless adult men, providing an array of services to its residents ensuring the dual goals of permanent housing and stable income are achieved.  This includes services such as case management, counseling, primary and behavioral health assessments, vocational training, entitlements, and others.  HOPE House will offer residents a positive and therapeutic environment during their stay. The overarching goal of HOPE House is to promote emotional, behavioral, and social development as well as enhance the self-esteem of the residents so that these men can transition to successful independent living in the community.
