Do you have billing questions or need further assistance?
To speak with someone about your bill, to pay your bill, or to make payment arrangements over the phone – please call one of LifeStream’s Billing Department Representatives at (352) 315-7500.

Health Plan Billing:

LifeStream accepts most major health plans, including Medicare and Medicaid. If LifeStream has received the information needed to bill your health plan, we will bill them for the services rendered on your behalf as a courtesy to you.

You are generally responsible for your co-pays, co-insurance, deductibles, and out-of-pocket expenses related to the services you receive from LifeStream.

While you will receive a copy of your statement promptly, it typically takes from 45-60 days for your insurance company to make a payment on your bill. We will be happy to share with you any information we receive from your insurance carrier. However, for answers about insurance payments, deductibles, and co-insurance payments, you will generally need to contact your insurance company directly.

Financial Assistance:

LifeStream is committed to excellence in providing quality behavioral healthcare while serving the needs of our community. If you need help paying your bill, Financial Assistance may be available to consumers who have limited or no resources to pay for emergent or medically necessary services rendered at LifeStream.

How Do I Qualify for Financial Assistance?

Financial assistance is based on information such as your yearly income, assets, and family size.

If a validated financial statement application reflects an income at or below 200% of the most current Federal Poverty Level for the stated family size, the account will likely qualify for financial assistance.

Hospital Price Transparency:

Pursuant to Section 2718(e) of the Public Health Service Act, LifeStream Behavioral Center’s Hospital Unit is required to make public our standard charges for all items and services provided by our hospital.

LifeStream’s Hospital Unit is defined as its 46-bed licensed, free-standing psychiatric hospital unit located at 2020 Tally Road in Leesburg, Florida within Lake County.

This disclosure contains all of LifeStream’s standard charges as well as other information required by the standards. Updated annually, any charges not specifically listed within our disclosure are items or services are presumably not currently offered or provided in our hospital unit setting.

We believe this disclosure allows our hospital consumers to make comparisons of common shoppable hospital services across health care settings.

The service bundle information is a non-anticipated services and that actual costs will be based on services actually provided to the patient.

LifeStream Behavioral Center is committed to price transparency and offers price estimates to prospective consumers through our Billing Department by contacting us at (352) 315-7500.

LifeStream may employ and/or contract with healthcare practitioners to provide services within its hospital.

Those practitioners may include the following:

Contact the health care practitioners anticipated to provide services to the patient while in the center regarding a personalized estimate, billing practices and participation with the patient’s insurance provider or health maintenance organization (HMO) as the practitioners may not participate with the same health insurers or HMO as the hospital and they will bill separately from the hospital.
